Launched by the InvestNorth Tulsa initiative, Northside Neighbors is a resident-driven nonprofit dedicated to fostering opportunity and prosperity in North Tulsa, by operating in four key areas: Mixed-income Housing, Cradle-to-Career, Community Wellness, and Economic Vitality. Position Purpose

The Neighborhood Community Health Worker (CHW) serves as a trusted liaison between the clinic and the community, working both on-site in a clinical setting and in the field throughout underserved neighborhoods. This role is critical in improving access to care, increasing health literacy, and advancing health outcomes for residents in North Tulsa—particularly within the Hawthorne and Walt Whitman communities.

The CHW supports patients directly within the clinic while also engaging residents in the community to promote available services, reduce barriers to care, and address social determinants of health.

Responsibilities/Essential Duties

  • Assist patients in navigating the healthcare system, including scheduling appointments, understanding treatment plans, and accessing specialty care.
  • Support patients with insurance enrollment and navigation, including Medicaid applications.
  • Provide follow-up support to ensure continuity of care and adherence to care plans.
  • Maintain accurate, confidential documentation in compliance with HIPAA standards.
  • Conduct outreach in the community to increase awareness of clinic services and available health resources.
  • Build relationships with residents, community partners, and local organizations to strengthen trust and engagement.
  • Identify barriers to care and connect residents to appropriate services and supports.
  • Develop and implement programs aimed at reducing chronic disease diagnoses and mortality.
  • Deliver a tobacco prevention curriculum to 4th-grade students in two elementary schools. Promote smoking cessation resources, including the 1-800 Tobacco Quit Line.
  • Provide health education on topics such as nutrition, physical activity, chronic disease prevention, and overall wellness.
  • Assist in the design, implementation, and evaluation of community health initiatives.
  • Track outreach efforts, patient interactions, and program outcomes using databases and reporting tools.
  • Support data-informed strategies to improve health outcomes in the target neighborhoods..
  • Perform other duties as assigned.

Education And Experience

  • High school diploma or GED required. Associate’s or Bachelor’s degree in public health, social work, health education, or a related field preferred.
  • Community Health Worker (CHW) certification (or willingness to obtain within a specified timeframe) strongly preferred.
  • Previous experience working in a healthcare setting, community-based organization, or public health initiative.
  • Experience delivering health education or youth programming.
  • Experience working with neighborhood residents in the field.
  • Knowledge of Medicaid systems, social services, and local resource networks.

Skills And Abilities

  • Community Expertise: Deep familiarity with North Tulsa resources and the specific needs of the Hawthorne and Walt Whitman neighborhoods, or similar lived experience.
  • Communication: Strong interpersonal and presentation skills; comfortable engaging with diverse audiences ranging from elementary school students to executive leadership.
  • Organization: Skilled in case management, documentation, and maintaining confidential records (HIPAA compliance); strong organizational skills with experience in database management and activity tracking.
  • Ability to build trust and establish relationships within underserved communities.
  • Cultural competence and a commitment to equity and inclusion.
  • Problem-solving skills and the ability to work independently and as part of a team.
  • Proficiency in basic computer applications and data systems.

Physical Requirements

  • Ability to lift up to 25 pounds on occasion (e.g., event materials).
  • Ability to stand, walk, or be physically active during community events or outreach activities.
  • Must be able to work both indoors (office setting) and outdoors (community events, neighborhood canvassing).
  • Must be able to travel locally and occasionally out of town as required.

Hours and Location

  • This is an in-person position based in Tulsa, Oklahoma.
  • Regular hours are Monday through Friday, 8:30 a.m. to 5:30 p.m.
  • Evening or weekend hours are required for events and community engagement.
  • Some travel is required for professional development, networking, and training.

Additional Requirements

  • Must have a valid driver’s license and active automobile insurance.
  • Must pass a background check prior to employment.
