Tufts Medical Center is a leading healthcare institution that is dedicated to providing exceptional care for transplant patients. Our Transplant Infectious Disease (ID) program plays a vital role in ensuring the best outcomes for patients undergoing solid organ transplant. Transplant ID is an integral part of the multidisciplinary services. Tufts performed the highest number of heart transplants in the Greater Boston area in 2024, and our program continues to grow in both volume and complexity. The transplant ID team includes ID physicians and an NP who collaborate with cardiology, nephrology, hepatology, surgical services, and hematology to manage all infectious disease aspects of care for pre- and post-transplant patients. We are looking for a LPN Navigator to join our team helping with transplant patient data management, laboratory tracking, coordination of patient appointments, schedules, direct patient calls, and help with care plans utilizing the nursing process and adhering to the standards of nursing practice. We are seeking a highly motivated and organized LPN to join our team and provide protocol-aligned clinical support and help strengthen our transplant outcomes and patient safety.

The Transplant ID LPN Navigator will serve as the primary point of contact for transplant patients regarding infectious disease management. This role involves coordinating care among the transplant ID team, multidisciplinary providers, and patients to ensure timely interventions, follow-up, and education. The LPN navigator will facilitate communication, track patient outcomes, and support quality improvement initiatives.

Job Description

Minimum Qualifications:

1. High school diploma or equivalent.

2. Graduation from an accredited practical nursing program.

3. Licensed Practical Nurse (LPN).

4. Two (2) years of relevant nursing experience.

Preferred Qualifications:

1. Three (3) years of relevant nursing experience.

Duties and Responsibilities: The duties and responsibilities listed below are intended to describe the general nature of work and are not intended to be an all-inclusive list. Other duties and responsibilities may be assigned.

1. Serve as the primary liaison between transplant patients, families, and transplant ID physicians.

2. Utilize the nursing process to deliver and manage the patient's experience through the continuum of care demonstrating diagnostic reasoning and critical thinking to ensure a seamless care service for the transplant patient and family.

3. Work with physicians and their staff to coordinate patient schedules through the system. Coordinate patient care by scheduling, directing calls, and ensuring timely access to services.

4. Manage and update transplant ID databases.

5. Track and maintain accurate and up-to-date patient records, including laboratory results, medications, and vaccines, and ensure documentation meets all standards.

6. Facilitate communication with external providers, pharmacies, laboratories, and shared care hospitals to ensure smooth care transitions.

7. Consult and refer patients to other resources and services as needed.

8. Serve as a patient and family advocate.

9. Implement interventions consistent with hospital standards of practice and prescribed medical care.

10. Documents the plan of care and meet all documentation standards.

11. Provide patient and family education regarding medications, monitoring responses under physician supervision.

12. Collaborate with outpatient shared care hospitals to ensure smooth care transitions.

13. Coordinate the appointments, workup, results of tests and other data necessary for the multidisciplinary conferences.

14. Delegate responsibilities to others with an understanding of their roles, knowledge and capabilities.

15. Facilitates cooperative and collaborative relationships among the various disciplines and departments to ensure effective quality patient care delivery and continuous flow of the patient's plan of care between all disciplines.

16. Maintain professionalism by demonstrating accountability, calm communication, and constructive participation in staff interactions.

17. Participate in quality improvement by gathering and analyzing regulatory data to support patient care outcomes.

Physical Requirements:

1. Prolonged, extensive, or considerable standing/walking.

2. Lifts, positions, pushes and/or transfer patients and equipment.

3. Considerable reaching, stooping, bending, kneeling, crouching.

4. Frequent exposure to hazardous chemicals, sick patients, bodily substances, noise and possible exposure to radiation, lasers, electric shock, etc.

5. Regularly exposed to the risk of bloodborne diseases and other transmissible infections.

6. Contact with patients under wide variety of circumstances.

7. Subject to varying and unpredictable situations.

8. Handle emergency and crisis situations.

9. Subjected to irregular hours.

10. May have contact with hazardous materials.

Skills & Abilities:

1. Ability to effectively communicate with patients, families, physicians, and healthcare team.

2. Knowledge and application of the nursing process to implement a nursing plan of care.

3. Possesses and applies the skills and knowledge necessary to provide care to patients throughout the life span, with consideration of aging processes, human development stages and cultural patterns in each step of the care process.

4. Ability to manage Excel, EMR, and data management.

Full time, 40 hours a week. The structure of the week may be negotiated. 4-10- hour shift vs. 5-8- hour shift
