Nature of Work:

The Diabetes Prevention Specialist is responsible for providing diabetes-related case management services and health education related to diabetes prevention and healthy lifestyles. The Specialist oversees the 2nd floor kitchen, maintaining safety of clients and consistent cleanliness of MNCR and kitchen, and coordinating health promotion and diabetes education events in the Community Room. Additionally, the Diabetes Prevention Specialist conducts program outreach and coordinates with other community health and social service agencies, including the TIC clinic. Works with individuals and groups to facilitate their use of TIC and other services.
Requirements:
Essential Functions:
- Provides individual case management services to clients addressing behavioral goals for a healthy lifestyle as relates to diabetes. Documents encounters in EHR.
- Designs, delivers and documents educational activities for youth, adults and elders regarding aspects of a healthy lifestyle, inclusive of diet, exercise, and diabetes prevention. Documents encounters into the EHR.
- Recruits and motivates urban Indian community members of all ages in weekly physical activity and health education.
- Oversees and monitors the day-to-day operations of the kitchen, maintaining safety, cleanliness, and standard business operating hours of 8:00 AM-5:00 PM.
- Conducts community outreach to attract clients for health promotion services.
- Develops educational and outreach materials that are appropriate for the Urban Indian community.
- Refers individual clients to enroll them in Wellness Department services, to include client intake, eligibility determination, and case plan development and case plan monitoring.
- Coordinates with TIC staff to coordinate the logistics for Community Room events, such as workshops and field trips.
- Establishes and maintains working relationships with community organizations and partners to coordinate services.
- Prepares required reports in an accurate and timely manner.
- Participates as part of the staff team of the Tucson Indian Center and Wellness Department.
- Participates in a professional development plan, as appropriate.
- Performs other tasks as assigned.
Required Experience and Training
- Bachelor’s Degree in health, nursing, or human services, such as public health, social work, or education. OR
- Associates Degree in nursing, human services, such as public health, social work, or education or Certified Care and Education Specialist and/or Certified Personal Trainer. OR
- A minimum of High School Diploma or equivalent (GED) that is a certified Care and Education Specialist and/or Certified Personal Trainer with combination of three (3) years of work experience.
Additional Requirements:
- Must possess valid Arizona Driver’s License
- Must pass Driver Insurance Carrier’s requirements
- Must obtain a HIPAA certification within 2 weeks of hire.
- Must obtain certifications in Cardio Pulmonary Resuscitation (CPR), Automated External Defibrillator (AED) and First Aid within 3 months of hire.
- Must pass Arizona DPS Fingerprint Clearance (Class I & II)
- Must pass and submit to periodic/random drug testing
Working Conditions:
- Exposure to communicable disease & bloodborne pathogens, unpredictable environmental conditions, physical requirements.
- Exposure to communicable diseases such as COVID-19 or bloodborne pathogens is a risk of this position.
- Unpredictable environmental conditions range from working in a climate-controlled office/clinic setting to working in a patient’s home, outside, in extreme temperatures, or in spaces with poor ventilation.
- While performing the duties of this job, the employee is regularly required to stand, walk, and drive in the Tucson Urban area.
- The employee is occasionally required to sit; balance; stoop, kneel, or crouch.
- The employee must occasionally lift and/or move up to 30 pounds.
