Job Overview
We are seeking a dedicated and compassionate Facilitator to join our team. The ideal candidate will play a crucial role in supporting individuals with various mental health and/or behavior challenges. The Facilitator will work closely with a multi-disciplinary team and must be organized, very flexible, and have the ability to coordinate and work well as a team member. The Facilitator will meet with individuals and families and must be able to develop interventions, direct team members and efficiently connect the individual/families to appropriate community-based resources.
Under general supervision, team with behavioral health professionals in planning and providing behavioral health services; provide general counseling, case management, community outreach, and prevention services.
The Facilitator promotes the mission of TTC4SUCCESS by ensuring that the team provides services in accordance with the wrap model and philosophy and in keeping with all policies and procedures.
Overall Description:
Under general supervision, team with behavioral health professionals in planning and providing behavioral health services; provide general counseling, case management, community outreach, and prevention services; and to do other work as required.
Incumbents in this class team with behavioral health professionals in the provision of student or participant treatment and prevention services, to directly assist students or participants in the utilization of appropriate community resources, and to provide education and information to the community.

The Facilitator serves as the primary contact point forTTC4SUCCESS, other service providers and other community partners and members.
Example of Job Duties:
- Provide high quality home/school/community-based mental health services or children and their families.
- Responsible for supervising the interdisciplinary family service team and ensures that services are provided in adherence with the service plan.
- Display professional skills in initial needs assessment (CANS), MTSS completion, case management, treatment planning, and utilization review.
- Act as leader of the Child & Family Team; convene the team and ensure continuity of care; ensure that all identified services are provided; ensure application of family-centered practice by the entire team.
- Serve as the contact point for children and their families, for service providers and for other members of the community; ensure continuous input and feedback from the family and service providers.
- Provide mental health services as needed.
- Visit students, individually or as part of a team, in a variety of living situations to help assess the mental health needs of individuals and families.
- Refer students to appropriate resources and/or makes appointment with behavioral health professional(s) for diagnosis and evaluation; assists the student, family, and other concerned individuals in filing for and obtaining various kinds of assistance such as financial, or housing.
- Teaches social skills, activities of daily living and other skills as necessary.
- Coordinate handling of child abuse and neglect cases through contact with school staff and outside agency personnel for the purpose of complying with mandated requirements.
- Documents all activities as required.
- Perform all other duties as assigned.
Minimum Job Qualifications and requirements:
- License/Certificate: Possession of a valid California Driver's License
- Proof of Car Insurance and Driving Record
- Successful completion of both a pre-employment exam, including a drug/alcohol and TB test, and a background investigation, which involves fingerprinting. (A felony or misdemeanor conviction may disqualify the applicant from employment)
- Bilingual preferred
- BA degree and/or 10 years’ experience in schools and or mental health, and/or working on hours for mental health licensure and/or drug and alcohol counselor certification with at least 5 years community or school experience,
- License or Credential preferred
Job Types: Full-time, Part-time
Pay: $24.00 - $28.00 per hour
