SHIFT HOURS: Hours vary per position (Ex. Mon to Fri 9am to 6pm / Mon to Fri 10am to 7pm; as needed)
DESCRIPTION:
The purpose of this program is to support and restore the delivery of clinical services that were impacted by the COVID-19 pandemic. Staff is responsible to provide new or enhanced outpatient services to individuals identified with serious emotional disturbance (SED), serious mental illness (SMI), and individuals with SMI or SED and substance use disorders, referred to as co-occurring disorder (COD) due to pandemic. Responsible for assessment of client needs for emergent, urgent and routine services. Provides skills/psychosocial rehabilitation, along with additional services such as COPSD, CAMS to clients authorized for such services.
At Center’s discretion, may be required to work from the office, in the community, or via telework to effectively accomplish regular case manager job duties. May be assigned to work or co-locate with community partners not limited to Law Enforcement, schools, hospitals, and community centers. Position may include cross coverage and assignment to serve in other programs to provide treatment services.
Examples of work performed
- Case management: Assists in accessing medical, social, educational, and other appropriate services.
- Provides Crisis Prevention and Management services
- Coordinates Services for In-patient Admissions for Psychiatric Services.
- Heavy concentration of Monitoring and Follow-up Services regarding individual needs of client.
- Provides Transportation of Clients in the Resolution of a Crisis.
- Coordination of Community Resources regarding the Specific Needs of the Client.
- Responsible for Medication Monitoring.
- Resolves Crisis Incidents in the Least Restrictive Measures.
- Provides psychosocial rehabilitation to clients during crisis assessments and follow up services
REQUIREMENTS:
Bachelor’s degree from an accredited college or University with a major in social work, psychology, or a field to human behavior. Bilingual (English/Spanish) required. One (1) year experience in the Mental Health Field, SED/SMI, and/or Crisis Services, Preferred. Bilingual (English/Spanish) required.
Must become certified to complete the State approved Uniform Assessment for Adults within 90 days of hire and maintain that certification without lapse.
