- Developing and updating individualized support and services plans
- Conducting regular contacts and in-person visits to monitor health, safety, and progress
- Promoting self-direction, independence, and community involvement
- Educating participants on program requirements and available resources
- Collaborating with community organizations, healthcare providers, and resource centers
- Maintaining accurate documentation and ensuring compliance
- Assisting with assessments and working independently within a support team
- Bachelor's degree in social services or related fields with relevant experience, or four years of direct service experience
- Strong communication, relationship-building, and cultural awareness skills
- Ability to work independently with a flexible schedule
- Proficiency with technology and transportation
- Fluency in additional languages (e.g., Spanish, Hmong, ASL) is highly valued
