Social Worker

We are seeking an experienced Licensed Social Worker to join our client’s team in Albany, NY. This role focuses on supporting children’s health and well-being through specialized programs and services.

📍 Location: Albany, NY

📅 Duration: 2 Years

🕒 Schedule: Monday–Friday, 8:00 AM – 5:30 PM

·                 Work with diverse populations, including children with serious emotional disturbances, developmental disabilities, medically fragile conditions, and substance use challenges.

·                 Gain expertise in programs such as Children’s Health Home, HCBS Waiver, and other state-supported services.

·                 Conduct HCBS eligibility assessments, manage referrals, and enroll participants.

·                 Assist with program planning, implementation, evaluation, and reporting.

·                 Develop policies, training materials, and maintain program updates.

·                 Collaborate with internal teams, contractors, and state partners to improve services.

·                 Experience: 6–8 years in social work or related field.

·                 Strong interpersonal, problem-solving, and time management skills.

·                 Knowledge of pediatric diagnoses, home health care, and Medicaid services.
