Position Summary

The Assistant Health Services Director directly oversees the delivery of care to Assisted Living residents to ensure that the emotional, physical, psychological, and safety needs are met through all program services. This includes medication management, staff coordination & training, infection control, coordination with outside healthcare providers, communication with families, and applying the nursing process to the care of seniors. Works within State and Federal regulations.

REPORTS TO: HEALTH SERVICES DIRECTOR

Position Responsibilities

  • MANAGEMENT
  • Adheres to and conveys the philosophy of supporting dignity, privacy, independence, choice, individuality, and a home-like environment for Residents.
  • Identifies ongoing needs and services for Residents through assessment and service planning utilizing the nursing process.
  • Coordinates, in cooperation with the Health Services Director, ancillary health care services for Residents.
  • Adheres to all established policies and procedures.
  • Responds to Resident or staff concerns and refers them to the Health Services Director when appropriate.
  • Manages and performs TB testing and screening for Residents and staff in accordance with state regulations.
  • Monitors the quality of work performance of Care Partners, Medication Staff, Licensed Nurses, and reports to the Health Services Director.
  • Ensures infection control procedures are known and followed by staff. Directs the infection control measures for the community.
  • Works closely with staff and designated agent(s) to identify Resident problems/concerns/issues; follows up as needed and communicates the same appropriately.
  • Oversees and/or completes monthly nursing schedule for nursing staff. Participates in the Manager on Duty program as directed by the Executive Director.
  • Coordinates/provides treatments within the scope of nursing license and state regulations.
  • Observes all work, safety, and administrative rules.
  • Clinical liaison with families, Residents, healthcare professionals, and team members.
  • Manage labor costs, supplies, and other line items in the clinical budget.
  • Develop strategies to eliminate/minimize the use of staffing agencies. Keeps the Health Services Director apprised of staffing agency usage.
  • RESIDENT SERVICES
  • Conducts initial nursing assessment(s)/evaluations to determine appropriate level of care. This may be done in a prospective Resident’s home or an alternative healthcare setting.
  • Conducts reassessments as per state/company requirements, including significant changes in condition, and prior to readmission to the community.
  • Evaluate personal services rendered to Residents and coaching staff on expectations
  • Coordinates the administration of medications and treatments, observing results and documenting them accordingly.
  • Addresses the physical, social, spiritual, and psychological needs of Residents.
  • Completes required forms to ensure continuity of care.
  • Notifies physician and/or designated agent(s) of Resident change in condition.
  • Performs other duties as assigned.
  • ADMINISTRATIVE
  • Reads staff communications and maintains familiarity with other Resident records as required.
  • Assures accurate transcription of physician orders into treatment/medication record.
  • Assists as directed with move-in and orientation of new Residents and designated agent(s).
  • Reviews Resident service records, treatment, and medication books to ensure the quality of care and documentation in the medical record meets nursing standards.
  • Verifies physician orders, move-in/move-out information, and initiates appropriate action/follow-up.
  • Supervises ordering of medications from the pharmacy.
  • Coordinates therapeutic diet orders with staff as required.
  • Assures proper records are maintained, including use of EHR systems for care documentation, medication management, and quality initiatives.
  • STAFF DEVELOPMENT
  • Provide, plan, schedule, and monitor staff training programs and in-services for the clinical staff, including statutory or regulatory required trainings.
  • Attend and complete all required training, in-service sessions, and staff meetings. Training of non-clinical staff may be required as needed.
  • Trains, documents, and supervises Care Partners under the Nurse Delegation Act as appropriate.
  • Training and supervision of staff to perform medication assistance services in accordance with best practices.

Employment Requirements

  • Willingness to adhere to assisted living principles in provided services.
  • Ability to relate to the public, Residents, families, staff, and other professionals appropriately.
  • Ability to communicate well, verbally and in writing.
  • CPR/First Aid Certified within 30 days of employment.
  • Ability to lift to 50 pounds occasionally, 30 pounds regularly.
  • Maintains appropriate and required nursing license in the state where the community is located.

Training And Experience

Licensed Nurse with at least one year of experience leading clinical operations in a senior living setting. Rehabilitative and restorative experience is helpful. CPR/First Aid certification.

Education And Licensure

Graduate of an accredited nursing school. Maintains current RN/LPN/LVN license in good standing in the state where the community is located. Immediately report to the Executive Director any disruption to your nursing license status. Provide the supervisor with all states where you hold an active nursing license/compact license.
