Job Overview
The Community Liaison serves as the bridge between our home health agency, referral sources, patients, and the community. This role is responsible for building strong relationships with healthcare providers, educating the community about available services, and generating referrals to support patient care and agency growth.
Conduct community outreach and provide education about home health services and eligibility.
Duties
- Serve as a trusted resource to referral sources, patients, and families regarding services, insurance coverage, and the referral process.
- Ensure smooth coordination of care from referral to admission.
- Work with clinical and intake staff to address barriers and ensure excellent patient experiences.
- Represent the agency at health fairs, senior centers, community events, and professional organizations.
- Deliver educational presentations and in-services to community partners.
- Promote the agency’s mission and values in the community.
- Maintain accurate records of outreach, referrals, and outcomes in CRM or reporting systems.
- Adhere to HIPAA and regulatory compliance requirements.
Experience
- Minimum of 2 years of marketing or sales experience in healthcare, preferably in home health, hospice, or post-acute care
- Strong understanding of Medicare-certified home health services, including eligibility and referral processes
- Proven ability to build and maintain relationships with referral sources (physicians, hospitals, case managers, and community partners)
- Excellent interpersonal, communication, and presentation skills
- Self-motivated with the ability to work independently, manage a territory, and meet performance targets
- Strong organizational skills with the ability to track outreach activities and follow up consistently
- Valid driver’s license and reliable transportation (local travel required)
- Bilingual (English/Spanish) preferred
- Experience with CRM systems or referral tracking tools
Pay: $50,000.00 - $70,000.00 per year
