Trusted Path Home Health is seeking an experienced Registered Nurse (RN) Case Manager to oversee and coordinate patient care for our growing Medicare-certified home health agency.

This is an office-based leadership role responsible for managing an active patient caseload, coordinating care with physicians and clinicians, and ensuring exceptional patient outcomes while maintaining Medicare compliance.

Responsibilities

• Manage an active Medicare home health patient caseload.
• Coordinate care between RNs, LVNs, PT, PTA, OT, ST, MSW, and Home Health Aides.
• Communicate with physicians to obtain verbal and written orders.
• Monitor visit frequencies and ensure timely patient care.
• Review clinical documentation for completeness and Medicare compliance.
• Coordinate recertifications, reassessments, discharges, and changes in patient condition.
• Serve as the primary clinical liaison between the office, field clinicians, physicians, patients, and families.
• Assist in maintaining quality measures and reducing hospital readmissions.
• Work closely with the Director of Nursing and clinical leadership team.

Requirements

• Current California Registered Nurse (RN) license in good standing.
• Minimum 2 years of Medicare Home Health experience (Required).
• Previous Home Health Case Management experience (Required).
• Experience using Synergy EMR (Strongly Preferred).
• Knowledge of OASIS, physician orders, Medicare Conditions of Participation, and home health documentation.
• Excellent organizational, communication, and multitasking skills.
• Ability to manage multiple priorities while maintaining exceptional attention to detail.

Preferred Qualifications

• Experience managing large home health patient caseloads.
• Strong physician communication and care coordination skills.
• Leadership experience in a Medicare-certified home health agency.
• Experience improving workflow efficiency and patient outcomes.
