**Experience:• 12 months of role experience is required with some in the last 12 months.
• 3 months of Home Health experience is required with some in the last 24 months.
• 3 months of experience with Homecare Homebase (HCHB) is required from any number of months.
Requirements:
• Candidates must have a California license (required for submission).
• Local & travel allowed.
• COVID vaccination required after submission. Religious and medical declinations accepted.

Additional Details:
Float experience does not count towards the unit experience requirement.
Monday-Friday
one weekend day per month rotation
Coverage area: Petaluma and surrounding
May be asked to help in other territories within facility's service area as needed which is about a 60 mile radius from the Facility address
Must have recent HH exp 2023-24
HomeCare HomeBase experience required, no exceptions
5 days maximum time off. Facility holidays: 12/25/2025, 12/24/2025, 12/31/2025, 11/27/2025. Number of holidays allowed off: 1
2 references from last 2 years (Manager/Supervisor reference type) - required for submission
Certifications:
• BLS (Basic Life Support)
