Additional Details:
Required Skills / Experience: 2 years of experience: in inpatient medical social work experience, with biopsychosocial assessment, assisting patients navigate the DCF process, collaborative care, care coordination with home care and equipment following discharge
Experience in rehab and long term care referrals, knowledge of community programs (AFDC, Medicaid, Food Stamps, Medication Assistance, and TANF), knowledge of protective services (adult and children protective services, emergency detention, guardianship and power of attorney)
Must have Medical Inpatient Experience
Adoption / Foster Care, Drug/alcohol use and treatment options, Knowledge of Protective services, Adult protective services, Child protective services, Emergency detention, Guardianship, Power of Attorney, Professional Knowledge and Skills, Advance Directives, Care Coordination to Coordinate services to home care and equipment following discharge, Documents pertinent information to facilitate care coordination, Participates in care conference activities, Refers to other disciplines/resources, Refers to rehab and long term care facilities), Supervises staff effectively and efficiently), Nursing Home, Discharge Planning, Work Settings (including Long Term Acute Care (LTAC), Rehab, Short Term Acute Care Inpatient Medical Social Worker)
Unit / Facility Details: Proof of Identification: Drivers License/State ID
Special Requests: References must cover at least 1 full year of relevant experience in specialty within the past 3 years.
Manager may or may not interview. Clinician must be okay with this. If Clinician has questions, please email the AM.
Shift & Scheduling: Monday - Friday
No on call requirement; traveler can pick up weekends if desired and available/they are needed
Weekend Coverage: Optional
Pre-Approved Time Off: 2 Requests
Floating Requirement: Float throughout patient tower across MedSurg units as necessary
