Position Overview
We are seeking a dedicated and compassionate Primary Care Nurse Practitioner (NP) or Physician Assistant (PA) to join our healthcare team. As a NP/PA specializing in primary care, you will play a crucial role in providing comprehensive medical services to patients. This position offers the opportunity to make a meaningful impact on the health and well-being of our community through patient-centered care and evidence-based practice working closely with a Primary Care Physician.
Scope of Responsibilities
- Conduct thorough physical examinations and health assessments.
- Diagnose and treat common acute illnesses and chronic conditions.
- Develop and implement personalized treatment plans in collaboration with patients and their families.
- Order and interpret diagnostic tests, such as lab work and imaging studies.
- Prescribe medications and other therapies as needed.
- Provide patient education on disease prevention, health promotion, and lifestyle modifications.
- Manage chronic conditions, including diabetes, hypertension, and asthma, with a focus on patient empowerment and self-management.
- Coordinate care with specialists and other healthcare professionals as necessary.
- Maintain accurate and up-to-date medical records using electronic health records (EHR) systems.
- Adhere to clinical practice guidelines and standards of care to ensure high-quality, safe, and effective patient care.
Minimum Qualifications
- Advanced Practice Registered Nurse licensure with concentration in Family Medicine or Physician Assistant licensure.
- Primary Care experience of 1 year required.
- Must be bilingual in both English and Spanish
- Significant complex problem-solving skills.
- Exceptional ability to care for others and engage in leadership functions in areas related to healthcare.
- Comfortable working independently.

- Works Monday through Friday 8:00 AM to 5:00 PM
- Work Environment – operates in a clinic environment.
Physical Demands
The physical essential functions of this job include (but are not limited to) the following: Frequently exerting 10 to 20 pounds of force to move objects; occasionally exerting up to 100 pounds of force. Physical demand requirements are in excess of those for sedentary work.
