Trueman Pointe Care Center, a skilled nursing facility in Hilliard, OH, is currently seeking a Social Services Designee to join our dynamic management team. The Social Services Designee is responsible for managing and/or assisting with various resident services including the resident waiver program, PASSAR requirements, Level of Care (LOC) and Medicaid eligibility requirements as well as discharge coordination, care conferences and assisting with quality assurance for resident services.
The successful candidate will have previous experience in a long-term care environment and exhibit a compassionate and empathetic demeanor in caring for our residents. License to practice Social Work (LSW) in the State of Ohio preferred.
