What are the responsibilities and job description for the Direct Care Provider position at True North Housing Alliance, Inc.?
Direct Care Providers are directly responsible for executing all tasks related to day-to-day and direct service operations. Direct Care Providers assist with intakes, bed assignment and coordination, daily living assistance, and skills development. Direct Care Providers complete tasks as assigned by Shelter Supervisors, Shelter Operations Manager, the Director of Operations, the AD and the ED. Direct Care Providers implement, oversee, and or coordinate guest access of specific Day Habilitation programs, including gardening, meal prep and assistance, daily chores (for able-bodied and willing guests), schedule coordination for clients whose work/appointments fall outside of normal check-in hours, and healthy communication and de-escalation skills practice. Direct Care Providers maintain up-to-date and annual training in self-monitoring, conflict-resolution, de-escalation, coping and grounding, Motivational Interviewing, and cultural competency.
True North pays $1.71 per hour for night differential in addition to the hourly rate.
Job Duties and Responsibilities
- Ensure the safety and security of all clients within the shelter environment.
- Maintain a respectful, non-judgmental, and trauma-informed approach in interactions with clients.
- Uphold the shelter’s rules and ensure compliance by clients, addressing violations when necessary.
- Collaborate with case managers, supervisors, and other staff to meet the needs of clients.
- Provide emotional support to clients, offering a listening ear and understanding of their situations.
- Communicate regularly with shift supervisors to report updates or concerns about clients.
- Maintain confidentiality and privacy of client information at all times.
- Participate in team meetings, training, and ongoing education to improve care and service delivery.
- Ensure common areas and dormitories are clean and welcoming for clients.
- Support the overall mission of True North Housing Alliance by promoting a safe and inclusive shelter environment.
- Respond promptly and effectively to emergencies and crisis situations, providing necessary intervention and support
- Adhere to all organizational policies, procedures, and regulatory requirements
- Ensure a safe, clean environment by following all health and safety guidelines
- Engage in ongoing professional development and training to enhance skills and knowledge
- Ensure task completion of assigned tasks
- Ensure clients have signed in nightly
- Maintain detailed and accurate records of clients’ care, including daily logs and incident reports
- Observe clients for any changes in behavior, health status, and mental health status, and report concerns in log and via email to case managers
- Have redirection conversations with clients in a professional manner
- Provide direct support and care to shelter clients, including assistance with daily needs and routines.
- Monitor the well-being of clients and report any concerns to the shift supervisor or case management team.
- Assist clients with check-in and check-out procedures, including documenting intake information.
- Distribute necessities such as toiletries, clothing, and bedding to clients.
- Maintain accurate records of client interactions and services provided.
- Conduct regular safety and wellness checks in dormitories and common areas.
- Assist with the setup and breakdown of meals, ensuring clients receive adequate portions when kitchen staff not present.
- Clean and sanitize common areas, including restrooms, lounges, and sleeping areas.
- Assist clients with laundry services, ensuring proper use of equipment.
- Monitor and support clients in adhering to shelter rules and regulations.
- Handle and document any prohibited items following the shelter's policies.
- Assist clients with accessing community resources and support services as needed.
- Address client grievances or refer them to appropriate personnel.
- Assist with distributing medications following established procedures.
- Respond to emergency situations, following established safety protocols.
- Hand out mail to clients
Education and Qualifications
- Bachelor's degree in social work, psychology, or a related field preferred.
- Minimum of 2 years of experience in outreach, case management, or a related field.
- Knowledge of the local homeless population, community resources, and social service systems.
- Strong communication, interpersonal, and organizational skills. Ability to work independently and as part of a team.
- Demonstrated ability to handle crisis situations and work with individuals who may have mental health or substance abuse issues.
- Reliable transportation.
