Family Medicine Physician (Outpatient Primary Care) — True Health NY

What you’ll do

• Provide full-spectrum outpatient primary care for adults and adolescents: wellness, chronic disease management (HTN, DM, HLD, COPD/asthma), women’s health basics, behavioral health integration, and urgent-care-level complaints.

• Close care gaps and optimize quality measures (preventive screenings, immunizations, chronic care targets) with strong MA/NP and central care-coordination support.

• Perform in-scope procedures as trained (e.g., EKG interpretation, simple skin procedures/cryotherapy, joint injections).

• Practice in eClinicalWorks (smart sets, templates, and macros provided) with <24-hour note closure and accurate coding.

• Collaborate cross-site (Fresh Meadows, Great Neck, Williston Park, Astoria, and Brooklyn) and escalate specialty needs through our referral network.

• Contribute to practice growth through exceptional patient experience, community reputation, and efficient access (same-/next-day slots).

Schedule & locations

• Full-time: 4x10s or 5x8s; one early or late shift weekly for access; optional 1 Saturday/month.

• 30-minute unpaid lunch; 2 hours/week protected admin time.

• Primary home site set at hire with occasional cross-coverage at nearby sites.

What success looks like (clear, credible goals)

• By months 3–4: consistent 20–24 patients/day with strong same-day access.

• ≥90% notes closed within 24 hours; ≥95% tasks closed within 48 hours.

• Quality gap closure at or above network benchmarks; patient experience ≥4.7/5.

• Accurate coding and documentation to reflect complexity and time.

What you’ll bring

• MD/DO; BC/BE in Family Medicine.

• Unrestricted NY license (or ability to obtain), DEA, BLS/ACLS.

• Comfort with eClinicalWorks (or quick learner) and team-based workflows.

• Professionalism, empathy, and crisp communication; second language (Spanish, Bengali, Mandarin, Russian, or Hindi) is a plus.
