Responsibilities
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
- Monitor and observe patient conditions, recording vital signs and reporting changes promptly
- Support medication reminders following proper procedures and documentation standards
- Utilize Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems to document care activities accurately
- Provide specialized care such as Alzheimer's, dementia, memory care, hospice, and working with individuals with developmental disabilities or disabilities
- Operate medical equipment like Hoyer lifts, catheters, and other assistive devices safely and effectively
- Maintain a clean and safe environment by assisting with light housekeeping tasks such as laundry, cleaning, and meal preparation
- Follow HIPAA guidelines to ensure patient confidentiality at all times
- Collaborate with healthcare teams to deliver comprehensive patient care tailored to individual needs
- Support clients in community settings by helping with transportation and community engagement activities
Qualifications
- Valid CNA certification with current active state licensure required
- Prior experience in assisted living facilities, nursing homes, or home health environments preferred at least 2 years
- Knowledge of medical terminology and clinical procedures such as catheter care and CPR certification required
- Experience working with seniors, individuals with disabilities, dementia or Alzheimer’s care highly desirable
- Ability to accurately monitor vital signs and perform patient observation
- Familiarity with EMR/EHR systems for medical documentation is a plus
- Strong communication skills to effectively interact with clients, families, and healthcare professionals
- Compassionate attitude with a commitment to providing respectful, patient-centered care
- Ability to work independently while following established protocols and safety standards
