Duties
- Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support
- Monitor and document patient vital signs such as blood pressure, pulse, temperature, and respiration rates using Electronic Health Record (EHR) systems
- Support medication administration following prescribed protocols while maintaining strict adherence to home healthcare regulations and HIPAA confidentiality standards
- Observe and report changes in patient condition or behavior, including patient observation for safety concerns or signs of deterioration
- Help with medical procedures such as catheter care, and assisting with end-of-life or hospice care needs
- Transfer patients safely using patient transfer methods like Hoyer lifts or other assistive devices; ensure proper body mechanics during heavy lifting or repositioning
- Perform light housekeeping duties including cleaning, laundry, meal preparation, and maintaining a safe environment for clients
Requirements
- Valid certification as a Certified Nursing Assistant (CNA) or Home Health Aide (HHA), with current state licensure if applicable
- Proven experience working with geriatric patients, individuals with developmental disabilities, dementia or Alzheimer’s care experience preferred
- Familiarity with EMR/EHR systems for accurate medical documentation and record keeping
- Knowledge of home health care assistance techniques including patient transfer methods and medication management
- Ability to communicate effectively with patients, families, and healthcare team members; compassionate caregiving mindset
- Basic life support (BLS) certification and first aid training required; CPR certification preferred
- Physical ability to perform tasks such as heavy lifting, assisting with mobility, and long periods of standing or walking
- Understanding of home health regulations, medical terminology, HIPAA compliance, and patient confidentiality standards
