Position Summary
Andrea's Angels is currently seeking multiple Per Diem Fee-for-Service Registered Nurses (RNs) to join our growing clinical team. This field-based position is responsible for providing skilled nursing visits and conducting in-home patient assessments throughout Greeley and Northglenn, Colorado, while ensuring patients receive high-quality, compassionate care.
We are seeking experienced, compassionate nurses looking for flexibility, autonomy, and the opportunity to make a meaningful impact in home healthcare. This role offers per-visit compensation and flexible scheduling.
Hiring for multiple locations: Greeley and Northglenn (Denver).
Compensation
Acute & Long-Term Home Health RN Visit Rates:
- One visit up to 2½ hours: $135
- RN Brief, 1st of Day (one visit): $90
- RN Brief, 2nd or later (one visit): $65
Compensation is fee-for-service and varies based on visit type and volume. This is a per diem position and is not eligible for employee benefits.
Essential Responsibilities
- Provide skilled nursing visits in accordance with the patient's plan of care.
- Conduct comprehensive in-home patient assessments, including Start of Care (SOC) and other clinical assessments as assigned.
- Accurately evaluate patients' medical conditions, functional status, and home care needs.
- Complete all required clinical documentation in a timely and accurate manner.
- Develop and update patient plans of care in accordance with agency policies and regulatory requirements.
- Collaborate with physicians, clinical leadership, Home Health Aides (HHAs), Personal Care Aides (PCAs), and the interdisciplinary care team to coordinate patient services.
- Educate patients and caregivers on disease management, medications, safety, and home care services.
- Report significant changes in patient condition to the appropriate clinical personnel.
- Ensure compliance with all agency, state, federal, and Department of Health regulations.
- Deliver compassionate, patient-centered care while maintaining professionalism during every patient interaction.
Qualifications
- Active Colorado Registered Nurse (RN) license in good standing is required.
- Minimum one (1) year of nursing experience preferred; home care experience is a plus.
- Strong clinical assessment, critical thinking, and documentation skills.
- Ability to work independently and effectively manage a field-based schedule.
- Willingness to travel throughout Colorado.
- Excellent interpersonal, communication, and organizational skills.
