We are seeking a Home Health & Hospice Liaison for a full-time, on-site position located in Los Angeles, CA. This role involves building and maintaining relationships within the community to promote True Care Health Services’ offerings in home health, caregiving, and hospice care. The Liaison will educate healthcare providers, facilities, and patients about our services, coordinate care transitions, and ensure patient needs are addressed. Additionally, the role involves identifying potential referral sources and working closely with the team to create personalized care plans.
- Experience in Hospice Care, End-of-Life Care, and Palliative Care
- Knowledge of Home Care services and the ability to communicate their value effectively
- Strong Sales and relationship-building skills, with the ability to engage healthcare professionals and community partners
- Excellent interpersonal and communication skills to educate and advocate for patients and families
- Ability to work collaboratively with an interdisciplinary team to coordinate patient care
- Previous experience in a healthcare or patient advocacy setting preferred
- Bachelor’s degree in healthcare administration, business, or a related field is a plus
- Valid driver’s license and reliable transportation for community outreach
