Position Summary
The Medical Coordinator is responsible for overseeing and coordinating all medical and healthcare services for individuals served. This role ensures that individuals receive appropriate medical care, medications are managed safely, and all healthcare documentation meets regulatory and organizational standards. The Medical Coordinator works closely with healthcare providers, staff, families, and interdisciplinary teams to support the overall health and well-being of individuals.
Essential Duties and Responsibilities
- Coordinate and schedule all medical, dental, and specialty appointments
- Ensure timely follow-up on physician recommendations and treatment plans
- Maintain accurate and up-to-date medical records for all individuals
- Monitor and document health conditions, changes, and concerns
- Oversee medication administration systems and ensure compliance with protocols
- Conduct regular medication and medical record audits
- Communicate with physicians, pharmacies, and other healthcare professionals
- Provide training and guidance to staff on medical-related procedures and protocols
- Ensure compliance with state regulations (including DBHDS standards if applicable)
- Assist in developing and updating individualized service plans related to medical needs
- Monitor infection control practices and health/safety standards
- Track and report incidents, hospitalizations, and significant medical events
- Support individuals in attending appointments and understanding their care
- Collaborate with guardians/family members regarding medical updates and needs
Qualifications
- High school diploma required; Certified Nursing Assistant, Medical Assistant or over 5 years or more in the healthcare field, relevant healthcare certification preferred.
- Experience working with individuals with intellectual and developmental disabilities (IDD) preferred.
- Knowledge of medication administration and healthcare coordination
- Familiarity with state regulations (e.g., DBHDS) and healthcare compliance standards
- Strong organizational, documentation, and communication skills
- Ability to maintain confidentiality and handle sensitive information
- Proficient in basic computer systems and electronic health records
Physical Requirements
- Ability to lift/move up to (specify weight, e.g., 25–50 lbs.)
- Ability to assist individuals with mobility as needed
- Frequent standing, walking, bending, and sitting
- Ability to respond to medical emergencies
Work Environment
- Residential group homes, day support programs, and community settings
- May require local travel to appointments and meetings
- Exposure to individuals with varying medical and behavioral needs
Additional Requirements
- Valid driver’s license and acceptable driving record
- CPR/First Aid certification (or ability to obtain)
- Medication Administration Training (as required by state)
- Background check and TB screening
Pay: $37,000.00 - $47,971.00 per year
