TruCare Community is an assisted living company for the elderly (ages 59 ) located in Bellflower, CA with facilties in the Long Beach, Stanton, and Bellflower area. We are currently seeking a Full-Time LVN/Program Coordinator to join our Wellness Department. Submit your application today!

Qualifications (non-negotiable):

Able to clear DOJ FBI live scan criminal background clearance mandated by licensing
Able to pass pre employment physical and TB test clearance
High school diploma or GED required
Flexible scheduling. Position may include weekends and holidays
Experience medication administration preferred
Must have valid LVN License (Required)

Job Description:

Assists Wellness Director with and maintaining Resident Assessments (RAF)/Individualized Service Plans (ISP) for each resident that is on a supplemental program. Communicates clearly with Administrator, residents and families any changes in condition or concerns, advising of changes in Level of Care, coordinates physician appointments, recommendations for Physical Therapy or Home Health Agencies.

Assist in coordination of resident and conversions onto appropriate supplemental programs, including paperwork, communication with program representatives, follow up, documentation, and related resident files. This includes program ISPs, Assessments, and acceptance documentation for the programs.

Assist in coordination of resident and admissions with appropriate supplemental programs, including paperwork, communication with program representatives, follow up, documentation, and related resident files.

Assists Wellness Director with audits and coordinating physician communications, medical updates, and pharmacy cycle changes and orders for residents on supplemental programs.

Assists Wellness Director with managing and maintaining all medication orders, inventory and supplies for each resident on a supplemental program, documenting information in appropriate resident records.
Additional duties as assigned.
