Registered Nurse (RN) – GAPP / Pediatric Home Care
TruCare 360 LLC is seeking a dependable, compassionate, and experienced Registered Nurse (RN) to join our growing pediatric home care team. This position will initially involve providing skilled nursing services for a GAPP patient and providing nursing oversight and support for the patient’s caregiver. The RN will also assist with new patient intakes, nursing assessments, care plans, caregiver training, and ongoing clinical needs as additional GAPP patients are accepted.
Current Case –
The initial assignment will involve working with a pediatric GAPP patient currently approved for approximately 8 hours of skilled nursing services per week.
The RN will:
- Provide skilled nursing services according to the patient’s approved plan of care.
- Complete and maintain required nursing documentation and visit notes.
- Monitor the patient’s condition and report significant changes as appropriate.
- Develop, review, and update the nursing care plan as needed.
- Provide caregiver education and training.
- Provide nursing oversight and guidance to mother or family member, who will be providing the patient’s approved personal care services.
- Monitor the caregiver’s ability to safely perform assigned personal care tasks.
- Communicate with the parent/caregiver, physician(s), and TruCare 360 administration regarding the patient’s needs.
- Ensure documentation is completed accurately and in accordance with applicable GAPP requirements.
Additional Responsibilities
As TruCare 360 continues to grow, the RN will also assist with:
- New GAPP patient intakes and nursing assessments.
- Reviewing medical records and physician orders.
- Completing nursing care plans for new patients.
- Caregiver skills checklists and teaching plans.
- Caregiver education and competency training.
- Initial and ongoing nursing assessments.
- Coordination with parents, physicians, and other members of the care team.
- Reviewing patient documentation for completeness and accuracy.
- Providing clinical guidance to caregivers and agency staff.
- Assisting with the transition of newly approved patients into services.
- Participating in ongoing supervisory and follow-up nursing visits as required.
- Identifying changes in patient condition or care needs and communicating them appropriately.
- Supporting TruCare 360 with clinical compliance and quality-of-care initiatives.
Qualifications
- Current, unrestricted Georgia Registered Nurse (RN) license.
- Pediatric nursing experience preferred.
- Experience with medically fragile children or children with developmental disabilities preferred.
- GAPP experience is strongly preferred but not required for the right candidate.
- Knowledge of Georgia Medicaid/GAPP requirements is a plus.
- Strong assessment, documentation, communication, and organizational skills.
- Ability to work independently in patients’ homes.
- Reliable transportation and ability to travel to patient homes for assigned visits.
- Compassionate and professional approach when working with children and their families.
- Ability to train and communicate effectively with family caregivers.

Pay: $25.00 - $50.00 per hour
