Position Summary
The Case Manager is responsible for coordinating client care services, developing care plans, and ensuring clients receive appropriate support tailored to their needs. This role serves as a key liaison between clients, families, caregivers, and healthcare professionals.
Key Responsibilities
- Conduct initial client assessments and ongoing evaluations
- Develop and update individualized care plans
- Coordinate and schedule home care services
- Communicate regularly with clients, families, and caregivers
- Monitor client progress and ensure satisfaction with services
- Maintain accurate and timely documentation
- Ensure compliance with company policies and state regulations
- Address client concerns and resolve issues promptly
Qualifications
- Bachelor’s degree in Social Work, Healthcare Administration, Nursing, or related field (preferred)
- Previous experience in case management, home care, or healthcare setting
- Strong organizational and communication skills
- Ability to multitask and manage a caseload efficiently
- Knowledge of home care regulations and best practices
- Valid driver’s license and reliable transportation
