This position is Per Diem and is paid per visit completed.

Physical Therapist Job Duties at TRU Healthcare: 

• Collaborate with DOR: Work independently yet cooperatively with the Director of Rehab and Operations Manager to develop personalized care and treatment plans for clients in their home environments. 
• Comprehensive Client Assessment: Evaluate clients' muscle strength, mobility, gait, range of motion, and pain levels. Select and recommend appropriate treatments and assistive devices and provide clear instruction to family members and caregivers on their proper use. 
• Communicate Effectively: Keep the home healthcare team and providers informed about clients' responses to treatments and any changes in their conditions. 
• Initiate and Educate: Lead the initiation of physical therapy programs and mentor other personnel, including family members and caregivers, in specific phases of physical therapy. 
• Documentation and Planning: Prepare detailed clinical and progress summaries, and actively participate in discharge planning. Submit copies of these summaries to the Director of Nursing to maintain accurate and up-to-date client records. 
• Respect and Courtesy: Treat all clients, families, and staff members with the utmost courtesy and respect.  

Job Requirements: 

• Valid MN PT license.
• Preferably experienced in home health and/or outpatient settings as a Physical Therapist.
• Ability to travel in the Grand Rapids and surrounding areas for patient care.
• CPR/BLS Certification.
