Responsibilities
- Assist residents with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and toileting to ensure comfort and dignity.
- Monitor residents’ vital signs and overall health status, reporting any changes or concerns promptly.
- Administer medications accurately following prescribed protocols and document all medication administration in medical records / (EHR) systems.
- Support residents with mobility using equipment such as Hoyer lift/walkers/wheelchairs and assist with transfers safely. This involves some lifting requirements.
- Support residents with respiratory supports, such as oxygen tanks and tubing.
- Prepare nutritious meals aligned with dietary plans and assist residents during meal times to promote proper nutrition and monitoring for choking protocols.
- Engage residents in social activities, behavioral management strategies, and recreational programs to foster socialization and mental stimulation.
- Maintain a clean, safe environment by performing routine cleaning duties in accordance with health standards.
- Document incidents, shift reports, and resident progress thoroughly to ensure continuity of care.
- Drive residents for appointments and community activities in your personal vehicle, ensuring safety at all times.
- Collaborate with team members, healthcare professionals, families, and social workers to coordinate comprehensive support plans.
Experience
- Prior experience working in supported living settings, assisted living facilities, nursing homes, or personal care homes is highly preferred.
- Demonstrated knowledge of dementia care, Alzheimer’s care, memory care, and senior care best practices is preferred.
- Experience administering medications and monitoring vital signs within long-term or home & community care settings.
- Familiarity with EHR systems for documentation purposes.
- Proven ability to work effectively with individuals who have developmental disabilities or behavioral challenges.
- Previous group home experience or caregiving roles that involved assisting with ADLs and personal care tasks.
- Reliable vehicle with a valid driver’s license and vehicle insurance for transportation duties is required
- Some service locations may involve exposure to animals/pets such as dogs, cats, and caged reptiles.
