About the Role
Aging Life Care Managers, also known as Geriatric Care Managers, are experts in aging and elder care who coordinate comprehensive care and support for older adults and their families.
In this role, you will serve as an advocate, resource navigator, and trusted partner, helping clients and their families navigate the challenges of aging while promoting dignity, independence, and quality of life.
Our client population is predominantly older adults, although not exclusively. You will work closely with clients, families, physicians, healthcare providers, and community resources to coordinate appropriate, high-quality, and cost-effective services.
What You’ll Do

Assessment & Care Planning: Conduct comprehensive assessments of clients’ physical, psychological, social, environmental, and financial needs. Develop individualized, goal-oriented care plans that address current concerns and anticipate future needs.

Care Coordination: Arrange, monitor, and adjust services such as home care, medical providers, rehabilitation therapists, mental health professionals, and community programs. Facilitate communication among clients, families, and providers.

Client Advocacy: Advocate for clients’ best interests and ensure their rights, preferences, and wishes are respected. Assist clients and families with navigating healthcare systems, insurance, and related legal or financial matters as appropriate.

Crisis Intervention: Respond promptly to medical, behavioral, or social emergencies and help develop strategies to prevent or mitigate future crises.

Family Support & Education: Provide education, guidance, and emotional support to families making care decisions. Help facilitate effective communication and navigate conflicts related to a loved one’s care and well-being.

Ongoing Monitoring: Regularly evaluate the effectiveness of care plans and services, reassess client needs, and adjust plans as circumstances change.

Documentation: Maintain accurate and timely records and documentation in accordance with professional and industry standards.
What We’re Looking For

Bachelor’s or master’s degree in Social Work, Nursing, Gerontology, Counseling, or a related field required

Relevant Massachusetts licensure or certification, such as Licensed Social Worker, Registered Nurse, or Licensed Mental Health Counselor, preferred

5 years of experience working with older adults

Strong familiarity with Massachusetts healthcare systems, elder services, and local community resources

Ability and willingness to travel throughout the region for client visits

Strong communication, organization, advocacy, and relationship-building skills

Ability to work independently while collaborating effectively with families and multidisciplinary care teams
Compensation & Benefits

Please Note About Location & Travel
This is a hybrid/field-based position. While there is no physical office that employees are required to report to, this is not a fully remote role. The Aging Life Care Manager must be located in Massachusetts and available to travel throughout the region to meet with clients in their homes, retirement communities, assisted living and nursing facilities, hospitals, and other care settings as needed.
Mileage and travel expenses are reimbursed.
