BMT Long Term Follow Up Coordinator

As a BMT Long Term Follow Up Coordinator, your voice to influence patient care is valued and empowered at every turn –whether through open, collaborative relationships with your direct manager or more formal opportunities through hospital councils and national nursing initiatives. You'll help shape decisions that elevate both patient outcomes and the future of nursing.

Job Summary and Qualifications

• Coordinate care of the post-transplant patient in the outpatient setting.
• Monitors patient/family after transplantation for physical, emotional, psychosocial, and financial needs and intervene when appropriate.
• Responsible for long-term follow-up, which includes but is not limited to: periodic updates on patient condition, coordinating follow-up visits with Blood and Marrow Transplant physician, scheduling disease restaging (and other tests as needed) services.
• Maintain awareness of outstanding lab/imaging results and relay to provider for prompt follow up.
• Coordinate referrals to physical therapy, home health, and other specialties as needed.
• Coordinate survivorship efforts between transplant team and outside physicians, optimizing patient recovery and health maintenance after transplant.

Qualifications:

• Valid TN RN license
• 3-5 years oncology experience including stem cell transplant and or heme-malignancy, adult setting preferred
• Current BLS
• ONS Chemotherapy Certification preferred
• BMT CN required within 4 years of hire
• BSN or Master’s degree preferred but not required
• Apheresis experience, preferred but not required
