We are seeking a motivated, relationship-driven Community Liaison to grow awareness and referrals for our PACE program. This role is ideal for someone with healthcare sales, senior services, outreach, and provider relations experience. Develop and maintain referral relationships with hospitals, physicians, rehab centers, senior centers, and community organizations Educate referral sources, families, and community partners on program benefits Generate qualified referrals and support enrollment growth Conduct presentations, community events, and outreach activities Maintain territory plans and referral pipelines Collaborate with enrollment, clinical, and operations teams to ensure smooth participant transitions Minimum Qualifications: Bachelor’s degree with 2 years of experience in healthcare sales, outreach, business development, or community engagement. A combination of education and experience may be acceptable.
