Master Social Worker- Hospice

Our Medical Social Workers (MSWs) play a vital role in providing compassionate support, helping clients and their loved ones navigate challenges, and connecting them to resources that improve quality of life.

This position offers the opportunity to work closely with patients, families, physicians, and an interdisciplinary healthcare team—bringing comfort, guidance, and hope when it’s needed most.

As a Medical Social Worker, you’ll be a trusted resource and advocate for patients and families. Your responsibilities will include:

• Supporting Families: Helping patients and loved ones cope with personal, social, and emotional challenges related to illness, home care, or hospice.

• Guiding Care Planning: Working with nurses, physicians, and other team members to develop individualized plans of care and assist with safe discharge or transitions.

• Providing Counseling: Offering emotional support, guidance, and counseling to help families understand and follow medical recommendations.

• Connecting Resources: Linking patients and families to community services, financial support, or other resources that ensure safe and dignified care at home.

• Collaborating as a Team: Actively participating in case conferences and sharing insights that help create the best outcomes for patients.

• Staying Connected: Maintaining regular communication with patients, families, physicians, and supervisors to ensure everyone feels supported and informed.

  What We’re Looking For

  • A Master’s degree in Social Work (MSW) from an accredited program.

  • Current state licensure (or eligibility).

  • Previous experience in healthcare, hospice, or home health is a plus—but most importantly, a heart for helping others.

  • Strong communication, problem-solving, and advocacy skills.

  • The ability to work independently while thriving as part of a caring, collaborative team.
