What are the responsibilities and job description for the Shelter Case Manager position at Trinity Integrated?
Position Summary:
Trinity Integrated Care is seeking a resourceful, and organized Case Manager to support individuals experiencing homelessness who are also living with, or may qualify for, Serious Mental Illness (SMI). This position is dedicated to guiding individuals through the process of SMI determination, connecting them with behavioral and physical health resources, and navigating the path toward permanent supportive housing and stability.
The Case Manager will be embedded in shelters across the Phoenix valley, including Central Arizona Shelter Services (CASS).
Key Responsibilities:
- Conduct outreach and case management services at assigned shelter sites.
- Assist clients in initiating and completing the SMI Determination process through AHCCCS or local behavioral health providers.
- Strong computer and documentation skills
- Perform needs assessments and create individualized service plans.
- Connect clients to medical, behavioral health, and substance use treatment services.
- Coordinate with shelter staff, peer support, housing navigators, and other providers to reduce barriers to care and housing.
- Assist clients with obtaining vital documents (ID, birth certificate, social security card).
- Support clients in applying for benefits such as Medicaid (AHCCCS), SNAP, and SSI/SSDI.
Qualifications:
- 1–2 years experience working with individuals experiencing homelessness and/or the SMI population (required).
- Knowledge of AHCCCS, SMI determination process, and ALTCS.
- Strong organizational and problem-solving skills.
- Ability to work independently in fast-paced, high-needs environments.
- Bilingual (Spanish/English) is a plus.
- Valid AZ Driver’s License, fingerprint card and reliable transportation (required).
Work Environment:
- Mobile role with frequent travel to shelter sites and client meetings.
- Combination of field work and office documentation.
