Salary: This Social Worker will get about 15-20 hours per week, and supporting approx. 5-10 patients. Candidate will support the South Barrington / Northwest suburbs area.Job Summary:The Social Worker will provide social services under the direction of a Physician, to offer support to our patients and their families. Social work services will be based on the patients psychosocial assessment, and the patient and familys needs. The Social Worker will participate in the coordination of care with the Interdisciplinary Group (IDG) and participate in on-call rotation per Agency needs.Essential Functions of the Job:Assess patients to identify psychosocial, financial, environmental and community needs as evidenced by POC documentation, clinical records, team meeting minutes and community resource referrals.Leverage effective interpersonal relations and communication skills; facilitates the use of these skills by other team members to achieve desirable outcomes.Meet mandatory continuing education requirements of the agency/licensing board.Demonstrate commitment, professional growth and competency by maintaining a working knowledge of public and private eligibility standards and requirements.Promote Agency philosophy and administrative policies to ensure quality of care.Attend interdisciplinary team meetings, participate in the patient care planning process and collaborate with the Interdisciplinary Team to promote coordination of patient care.Accept clinical assignments that are consistent with education and competence to meet the needs of the patients.Education, Experience, and Certifications Needed:A Bachelors or Masters Degree in Social Work from a State accredited program. If the Social Worker has a Bachelors degree and is hired after 12/02/08, they must be supervised by a Masters of Social Work.One (1) year social worker experience working for a hospice agency is heavily preferred.Working knowledge of community resources.Exceptional interpersonal skills.Must have a current drivers license, reliable transportation and valid auto liability insurance.
