While we have a strong presence in skilled nursing facilities, we are looking to develop a more robust footprint within the areas/communities we serve.
Position Overview
We are seeking a motivated, relationship-driven liaison to help grow our census and expand our footprint across the South Shore, Upper Cape, and South Coast regions.
This role is ideal for someone who thrives on building genuine relationships, understands hospice referral dynamics, and wants to play a key role in scaling a high-quality, mission-driven hospice organization.
Key Responsibilities:
-Community Engagement: Represent Trinity Hospice at community events, networking opportunities, and industry functions
-Develop and maintain strong referral relationships with:
- Skilled nursing facilities
- Assisted living communities
- Hospitals and case managers
- Physicians and community providers
-Identify and pursue new growth opportunities, particularly in home-based hospice referrals
- Track and manage referral activity using internal tools (Excel/CRM-style tracking)
- Collaborate closely with clinical leadership to ensure smooth patient transitions and high-quality care delivery
-Provide education to referral partners on hospice eligibility, services, and differentiators
-Help shape and execute strategic growth initiatives

Qualifications
- 2 years of healthcare sales, liaison, or business development experience (hospice, home health, SNF, or related preferred)
- Strong understanding of hospice referral networks and discharge planning
- Excellent interpersonal and communication skills
- Self-starter with the ability to work independently
- Organized and comfortable tracking data, outreach, and follow-ups
- Reliable transportation and willingness to travel locally
