Qualifications - Registered Nurse (RN):
- Prefer 1 year Home Health experience.
- Must be able to work a flexible schedule.
- Must have dependable transportation, driver's license and car insurance.
- Current AR nursing license, TB and BLS
- Pass drug test and background check
Responsibilities - Registered Nurse (RN):
- Initiates and provides services in accordance with the plan of care.
- Performs Initial Oasis assessments, services requiring specialized nursing skills, and supervisory visits when necessary.
- Prepares clinical and progress notes for each patient visit and summaries of care conferences on his/her patients in a timely manner as per Trinity Home Health of Arkansas policy.
- Coordinates services & communicates with clinical manager/ PCP any changes in patients condition that requires adjustment to the plan of care or level of care.
- Understands and adheres to established Trinity Home Health of Arkansas policies and procedures.
- Completes certification/recertification orders and discharge summaries.
- Refers to Physical Therapist, Speech Language Pathologist, Occupational Therapist and Medical Social Worker those patients requiring their specialized skills.
- Supervises and teaches other nursing personnel.
- Attends Patient Care Conference and takes on-call duty nights, weekends, and holidays, as assigned.
- Completes and submits OASIS assessments, reassessments, transfers, resumptions of care, discharges, and significant change in condition in accordance with Trinity Home Health of Arkansas defined time frames.
WORKING ENVIRONMENT:
Works indoors in Trinity Home Health office and patient homes and travels to/from patient homes.

Pay: $35.00 - $40.00 per hour
