What are the responsibilities and job description for the Intake Coordinator/Scheduler position at Trinity Home Health?

This position requires computer skills, data entry, ability to multi-task, answer phones and filing among other clerical duties. We would prefer someone with medical, home health or hospice office experience but not required. LPN preferred but not required.

As an Intake Coordinator with Trinity Home Health you will manage all aspects of our referrals.

  • Coordinate daily patient referrals and intake operations.
  • Verify insurance eligibility.
  • Establish and maintain positive relationships with referrals and customers by responding to their requests and concerns in a timely manner.
  • Ensure seamless transition of patients to home health by providing coordination of staff for start of care and evaluation processes.

Job Type: Full-time

Pay: $22.00 - $25.00 per hour

Expected hours: 40 per week
