Duties
- Assist patients with activities of daily living such as bathing, dressing, grooming, and mobility support using equipment like Hoyer lifts and manual handling techniques.
- Monitor and record vital signs including blood pressure, pulse, temperature, and respiration rates accurately in electronic health record (EHR) systems.
- Support medication administration under supervision, ensuring proper documentation in compliance with HIPAA regulations.
- Provide specialized care for patients with dementia or Alzheimer’s disease, including memory care strategies and behavioral health support.
- Collaborate with nursing staff to implement individualized care plans for patients with complex needs such as critical care, ICU, trauma center, or pediatric cases.
- Maintain aseptic techniques during procedures like catheter care and wound management to prevent infection.
- Assist in patient transfers and positioning using safe lifting practices and equipment like Hoyer lifts.
- Document patient interactions thoroughly in medical records and electronic health records (EHR/Epic/eClinicalWorks), ensuring accuracy and confidentiality.
- Support patients in community-based settings or home environments by delivering compassionate home & community care services.
Requirements
- Valid CNA certification with current licensure in the applicable state.
- Proven experience working with diverse populations such as geriatrics, pediatrics, or individuals with disabilities.
- Knowledge of medical terminology, physiology, and anatomy relevant to patient care.
- Hands-on experience in medical-surgical units, long-term care facilities, hospice or palliative care settings, or emergency medicine environments such as Level I/II trauma centers or ICU units.
- Familiarity with EMR/EHR systems like Cerner or eClinicalWorks for accurate documentation.
- Ability to perform heavy lifting safely and adhere to safe lifting practices during patient transfers.
- Strong communication skills to effectively collaborate with multidisciplinary teams and provide excellent patient service.
- CPR/BLS certification required; additional certifications such as phlebotomy or medication administration are a plus.
- Demonstrated commitment to infection control protocols including aseptic technique and universal precautions.
