Location: Saint Francis LIFE College Avenue, Newark, DE
Position Purpose
Coordinates and schedules outside medical appointments for PACE participants, maintains accurate medical records, and works closely with transportation, caregivers, and center staff to support participant care plans and compliance requirements.
Position Details
May require travel to site in Wilmington.

Position Purpose
Coordinates and schedules outside medical appointments for PACE participants, maintains accurate medical records, and works closely with transportation, caregivers, and center staff to support participant care plans and compliance requirements.

What You Will Do
- Maintains accurate participant schedules in alignment with plans of care and documents all updates.
- Coordinates appointments across departments and communicates required changes to providers.
- Uses electronic scheduling systems to schedule services as directed by the IDT and Nursing leadership.
- Provides appointment reminders and supports timely service delivery and participant satisfaction.
- Collaborates with clinical leaders to ensure scheduling accuracy and completion of participant education.
- Organizes, maintains, and protects electronic and paper medical records in regulatory compliance.
- Requests, tracks, uploads, and responds to medical record requests using secure processes.
- Participates in required training, surveys, compliance activities, and other assigned projects.
- Associate’s degree in applied science or related field.
- 2 years scheduling and medical records experience.
- Equivalent combination of education and experience may be considered.
- Strong judgment, business acumen, adaptability, and ability to work independently.
- Excellent written and verbal communication; effective with diverse audiences.
- Advanced Microsoft Office proficiency; experience with scheduling systems and strong typing skills.
- Highly organized, customer-focused, and able to manage multiple priorities and interruptions.
- Proven problem-solving, analytical, and critical-thinking skills with strong accountability.
