St Mary is a member of Trinity Health PACE. Programs of All-Inclusive Care for the Elderly (PACE) or LIFE programs coordinate and provide all needed preventive, primary, acute and long-term care services so that older individuals can continue living in the community.
Many PACE participants get most of their care from staff employed by the
PACE organization in the PACE center. PACE centers meet State and Federal safety requirements and include adult day programs, medical clinics, activities, and occupational and physical therapy facilities.

What You Will Do
- Provide primary medical coverage in collaboration with PO physicians as needed. Performs initial, semi-annual and annual assessments, performs additional assessments of participants as needed and implements appropriate plans of care to PO participants.
- Participates in discharge planning for acute and long-term placement.
- Evaluate and treat participants during acute illness. Prudently prescribes medications, therapies and other treatments for participants. Following consultation with the Primary Care Physician refers participants to medical specialist as indicated.
- Make nursing home/or home visits when needed.
- Work with Medical Director and Clinic Supervisor to formulate clinical policies, procedures and standards of care. Assists with the development of standards of care; performs on-going monitoring and evaluation of patient care practice and service delivery; provides guidance and training to staff regarding medical and quality assurance issues.
- Advise the Medical Director and Primary Care Provider in ways and means to establish better accountability of the PO services to participants and referral sources. Acts as resource during intake of new participants and in the day-to-day operation of the center programs.
- Participate in provider on-call rotation as required.
- Master's degree in Nursing
- Five (5) to seven (7) years of experience required preferably in long-term care.
- Must possess a current, unencumbered, active license to practice in the state of PA.
- CDS Certification and DEA Registration required.
- Board Certification by ANCC or AANP.
- Certification as a Nurse Practitioner with prescriptive authority required.
- Current BLS required.
- A minimum of one (1) year working with the frail or elderly population strongly preferred
- Must be action-oriented, be customer focused, have high decision quality, flexibility to adapt to ongoing change and have organizational agility.
- Must have a valid driver’s license, proof of insurance and have means of transportation
- Proficiency with Microsoft product suite (MS Word, Excel, Power Point, etc.); basic knowledge and experience with electronic mail and calendaring system
- Position requires periodic travel within the Centers geographic region.
- Position requires frequent kneeling, crouching, reaching, lifting, sitting, walking, bending and lifting. Must be able to push, pull, move and or lift a minimum of 20 pounds if applicable.
