What You'll Do:
- Provide social work services during the 8:00 AM – 4:30 PM shift in the inpatient hospital on the weekends only
- Conduct thorough psychosocial assessments for patients and families in crisis.
- Develop individualized treatment and discharge plans that address emotional, social, and practical needs.
- Deliver brief counseling and crisis intervention.
- Coordinate with community agencies and internal resources to support safe care transitions.
- Serve as a patient advocate and liaison between care teams and families.
- Document assessments, plans, interventions, and outcomes in a timely and accurate manner.
- Collaborate with the interdisciplinary care team to support holistic patient care.
- Education: Master’s Degree in Social Work or a related field from an accredited institution.
- Licensure: Eligible for licensure as a Limited Licensed Master Social Worker (LLMSW) or Licensed Master Social Worker (LMSW) by the Michigan Board of Social Work.
- Experience: Minimum of 2 years of social work experience in a healthcare setting; experience in emergency, behavioral health crisis, or acute hospital preferred.
- Exceptional crisis intervention and psychosocial assessment skills.
- Strong emotional resilience and sound decision-making under pressure.
- Compassionate communication and active listening.
- Collaborative team player who thrives in a dynamic, high-acuity environment.
- Familiarity with local resources and referral processes for at-risk populations.
- Accurate, timely clinical documentation and case tracking.
