- Provide skilled nursing care in accordance with physician orders and organizational policies
- Perform comprehensive assessments and develop individualized care plans
- Collaborate with physicians, therapists, social workers, and home health aides
- Supervise and support LPNs and Home Health Aides
- Ensure compliance with regulatory standards and contribute to quality improvement initiatives
- Promote patient satisfaction and positive health outcomes
- Current RN license (BSN preferred)
- Minimum 1 year of nursing experience (required in California)
- Home healthcare experience preferred
- Valid driver’s license and reliable transportation
- Strong computer skills and a commitment to ethical, mission-driven care

- Frequently lift, stand, and walk (35% of time)
- Occasionally bend, stoop, twist, and assist with patient positioning (10%)
- Independently lift up to 35 lbs
- Perform detailed tasks with frequent interruptions and shifting priorities
- Adapt to changing work environments and travel to various work locations
