- Education: Bachelor's degree with progressive experience or a Master's degree in Physical Therapy.
- Licensure: Must be licensed to practice as a Physical Therapist in Illinois.
- Experience: Previous experience in home healthcare or familiarity with conditions of participation is preferred.
- Additional: Must have a current driver’s license and reliable transportation to travel to and from patient sites.
