Job Title: Contingent Speech-Language Pathologist (Home Health)

Summary:
Provides professional speech and audiology diagnostic and therapeutic services to clients as prescribed by a physician, in compliance with state practice acts and licensure requirements. Coordinates care, develops individualized treatment plans, and promotes client independence through effective communication and rehabilitative techniques.
Key Responsibilities:
Assess, plan, and deliver speech-language pathology services in home health settings
Develop and revise care plans in collaboration with physicians and clients
Educate clients and caregivers on treatment and recovery techniques
Document clinical findings, progress, and updates in accordance with agency policy
Participate in quality improvement and interdisciplinary coordination
Supervise HHAs and mentor new staff as assigned
- Meets the educational and experience requirements for a Certificate of Clinical Competence in Speech
requirements for certification and is in the process of accumulating the supervised experience required for
certificate.
- Registration or license to practice as a speech pathologist in the state of practice if applicable.
- Must accept responsibility for maintaining clinical practice skills, learning and adhering to agency’s policies
- Must be able to function in a practice environment with minimal direct supervision, accepting personal
- Must have current Driver’s license and reliable transportation to and from work site.
- Accurately documents observations, evaluations and interventions pertaining to client care management
This will include assessments, orders, visit reports, medication information, treatments, coordination of
services, client progress or lack of progress toward goals, and contacts with physicians and agency and
community providers as appropriate.
- Takes appropriate steps to assure timely submission of client data to the office for inclusion in the client
- Works with scheduling to assure visits and client care coverage are scheduled in accordance with the plan

Mental and Physical Requirements • Lifts, stands, and walks 10% of time. • Bends, stoops, and twists 10% of time. • Positions and lifts patients 10% of time. • Ability to perform continuously detailed tasks with frequent interruptions and work reprioritizations (90% of time). • Must be able to adapt to frequently changing work parameters • Must be able to travel to various TH/THHS worksites or various work locations as required
