Your Role

Your Role
As a Physical Therapist, you’ll deliver professional, evidence-based care in accordance with physician orders, state regulations, and agency policies. You’ll help clients regain independence and improve their quality of life through personalized rehabilitation plans.
Key Responsibilities
- Conduct comprehensive assessments and develop individualized treatment plans.
- Deliver therapeutic interventions, including manual therapy, modalities, and exercise programs.
- Educate clients, families, and caregivers on therapy techniques and home programs.
- Supervise Physical Therapist Assistants (PTAs) and Home Health Aides (as permitted).
- Collaborate with interdisciplinary teams and participate in care conferences.
- Monitor progress, adjust treatment plans, and communicate changes to physicians.
- Maintain accurate, timely documentation in compliance with regulations.

- Degree from an APTA-accredited Physical Therapy program
- Active state license to practice as a Physical Therapist
- 1 year of experience or completion of a 6–8 week preceptorship
- Home health experience and knowledge of Medicare Conditions of Participation preferred
- Valid driver’s license and reliable transportation
- Strong organizational and time-management skills
- Ability to lift up to 35 lbs and assist with patient mobility
- Frequent standing, walking, bending, and positioning
- Adaptability to changing schedules and environments
- Travel to client homes and Trinity Health locations required
