Position Summary
The Family Support/Early Childhood Specialist is responsible for ongoing management for an average of 15 children and families. This includes goal planning, development, and keeping case records. This position also involves the planning and advocacy of services to assist families and administering the Ages and Stages Questionnaire Screenings as indicated and other responsibilities as designated.
Key Responsibilities:
Case Management & Family Support
Responsible for a caseload of children and their families to plan long and short-term goals.
Conduct weekly home visits.
Develop and keep a weekly case record of worker, parent, and child interactions and indicate how the objectives are being met.
Administer timely developmental screenings for program participants.
Encourage parent participation in program and agency activities.
Conduct Community Outreach.
Function as a mandated Child Abuse and Neglect Reporter.
Prepare all release and consent forms.
Responsible for submitting all necessary statistical data and forms required by funding sources.
Administer participant satisfaction surveys.
Write brief weekly reports and quarterly reports for the Program Supervisor.
Ensure timely completion and submission of all required documentation.
Collaborate with agency partners and community service providers to enhance program effectiveness for families.
Participate in staff development opportunities and in-service training and contribute to the program's continuous improvement.
Foster Care Prevention Program Collaboration
Attend Intake Meetings and Service Plan Reviews
Conduct bi-weekly home visits.
Ensure all Prevention families receive ages and stages screening.
Participate in ongoing case management, planning, and advocacy services for families.
Participating in client case reviews to serve as an advocate for clients in obtaining and/or maintaining supportive services.
Keep staff informed of family program status.
Responsible for the timely completion and submission of all required documentation including progress notes, FASP case updates and discharge summaries.
Assume other job-related duties as assigned by the Program Supervisor, CNS Division Leader, and/or Agency CEO.
Qualifications:
BS in Education and/or Human Services preferred or an AA Degree with related experience working with young children and their families.
Knowledge and experience working in the field of Developmental Disabilities is preferred.
Considerable work experience and professional training may be substituted for the educational requirement.
Use of own vehicle.
Valid New York State Driver's License and in good standing.

Salary: Hourly $24.67 (range $38,000 - $45,000)
