Job Overview
We are seeking a dedicated and experienced Hospice Registered Nurse (RN) Visit Nurse to deliver high-quality, compassionate care directly to patients in their homes. The ideal candidate will possess extensive clinical expertise, including critical care, ICU, emergency medicine, and post-acute care experience. As a Hospice RN Visit Nurse, you will play a vital role in managing patient needs, coordinating care plans, and ensuring comfort and dignity for our patients during their end-of-life journey.
Qualification:
· RN with Current Minnesota License
· Minimum 2 years of experience in Geriatrics, Home Care or Hospice

Hospice Clinical Manager

· Maintain the vision, mission, and values of the organization.
· Remain up-to-date regarding agency policies and procedures and perform all functions in accordance with Triniti policies and procedures
· Provide the highest quality of customer service consistent with Triniti values and standards.
· Maintain client confidentiality always as per the Health Insurance Portability and Accountability Act (HIPAA).

Essential Functions:
1. Attendance and reliability for undisrupted care and services to clients
2. May provide skilled nursing care to Hospice patients
3. Provide skilled nursing care interventions according to the individual care plan as developed by the agency RN case manager or ordered by a licensed provider.
4. Report changes in health status, responses to interventions, and any new orders received to the RN Case Manager timely
5. Monitors client’s vital signs, and general health status / condition.
6. Obtains and implements physician orders
7. Reorders meds as needed
8. Communicates to RN Case Manager any changes in the client's condition and/or orders
9. Collaborates with family and other professionals to best coordinate care
10. Maintains and completes accurate documentation in a timely manner:
a. Point of Care documentation will be completed with the expectation that at least 80% of the documentation is done in the home
b. Subsequent visits should be completed and synced on the same day, with less than 15 minutes of additional documentation time.
c. Utilize IO Health tool as able to deliver exceptional documentation
11. Schedules/completes visits during regular business hours and per weekend/holiday rotation schedule
13. Participates in case conferences and/or IDT meetings if applicable
14. Attends staff meetings and educational programs as required
15. Follows State and Federal Regulations for Home Care and Hospice
16. Other duties as assigned
Reliable Transpiration Necessary
Work Environment:
· Requires daily travel by automobile in all weather conditions
· Occasional exposure to unclean or unsanitary conditions during course of home visits including cats, dogs, smokers.
· Smoke free office
Physical Demands and Working Conditions:
Ability to lift fifty (50) pounds occasionally. Moving, lifting or transferring of clients may involve lifting to 50 pounds less than occasionally. Ability to bend, stoop, stretch, squat, twist, sit and reach. Fine motor skills and viewing of digital monitor.
