What You'll Do:

• Coordinate the intake of patients referrals and ensure timely admission to home health services
• Communicate effectively with agency staff and external partners
• Manage multiple tasks and projects with professionalism and accuracy

What You'll Bring:

• High School Diploma; some college preferred
• 1 years of experience in a similar role, Medicare Home Health experience preferred
• Organized team player with a positive attitude and strong communication skills
• Ability to multitask and stay detail-oriented in a fast paced environment
• Comfortable using MS Office, Excel, EMR systems, and company websites
