• Coordinate and manage schedules for patient visits, ensuring proper team assignments
• Record and communicate schedules with staff and clinical team members

What You Bring:

• High school diploma, or equivalent.
• Two (2) years of work experience in home health care or related health program preferred.
• Excellent communication and organization skills.
• Ability to multitask, ensuring the completion of several tasks, projects, and reports throughout the day.
• Intermediate functional knowledge of computer/technology skills including, but not limited to MS Office, MS Excel, navigation of company website, and electronic medical record.
