- Coordinate the intake of patients referrals and ensure timely admission to home health services
- Communicate effectively with agency staff and external partners
- Manage multiple tasks and projects with professionalism and accuracy

- Current and active LPN license in the state of practice
- 1 years of experience in a similar role, Medicare Home Health experience preferred
