Trillium Wellness Center is a 16-bed adult residential mental health facility located in Seymour, Tennessee. We provide individualized care in a calm, compassionate, and supportive environment.

We are seeking an experienced and dependable licensed nurse to serve as our Assistant Director of Nursing (ADON). Both Licensed Practical Nurses (LPNs) and Registered Nurses (RNs) are encouraged to apply.

This is a working leadership position that combines nursing-team management, scheduling, medication oversight, administrative responsibilities, and direct client care. The ADON will report to and work closely with the Director of Nursing.

Primary Responsibilities

  • Assist the Director of Nursing with the daily operation of the nursing department
  • Create, maintain, and communicate nursing schedules
  • Monitor staffing needs and help ensure appropriate nursing coverage for all shifts
  • Respond to nursing call-outs and actively assist with finding replacement coverage
  • Maintain an updated list of PRN and available nursing staff
  • Communicate uncovered shifts, staffing concerns, and schedule changes to the DON
  • Assist with interviewing, hiring, onboarding, orientation, and training of nursing staff
  • Help train nurses on medication procedures, documentation requirements, safety protocols, emergency procedures, and daily workflows
  • Provide routine direction, support, and coaching to nursing staff
  • Monitor attendance, punctuality, documentation, and compliance with nursing expectations
  • Report clinical, performance, conduct, or safety concerns to the DON
  • Assist with nursing staff meetings, education, and competency reviews
  • Participate in multidisciplinary treatment-team meetings
  • Assist with preparation for state surveys, licensing visits, internal audits, and corrective-action plans
  • Help maintain required nursing licenses, CPR certifications, training records, and personnel documentation

Medication and Quality-Assurance Responsibilities

  • Audit medication administration records for accuracy, completeness, and missing documentation
  • Help maintain an organized, clean, secure, and compliant medication room
  • Monitor medication inventory, refills, pharmacy deliveries, discontinued medications, and expired medications
  • Coordinate routine medication needs with pharmacies and prescribing providers
  • Assist with controlled-substance counts and immediately report discrepancies
  • Review admission and discharge medication reconciliation for completeness
  • Track medication errors, missed doses, adverse reactions, and required follow-up
  • Ensure provider orders are properly received, documented, communicated, and implemented
  • Audit nursing notes, intake documentation, vital signs, rounds, shift reports, and other required records
  • Identify documentation or workflow concerns and communicate them to the DON
  • Maintain adequate nursing supplies, first-aid materials, and emergency equipment

Direct-Care Nursing Responsibilities

  • Work scheduled nursing shifts and assist with coverage when reasonably needed
  • Administer scheduled and PRN medications according to provider orders
  • Complete nursing portions of client admissions and discharges
  • Obtain and document vital signs, weights, blood glucose readings, and other ordered measurements
  • Conduct nursing and safety rounds throughout the facility
  • Monitor clients for medication effectiveness, side effects, adverse reactions, and changes in condition
  • Observe clients’ physical health, mental status, behavior, sleep, and overall functioning
  • Promptly report changes in condition or safety concerns to the DON or appropriate provider
  • Provide medication education and general wellness education to clients
  • Assist with crisis response, de-escalation, first aid, and emergency procedures
  • Coordinate outside medical appointments, laboratory services, pharmacy needs, and follow-up care
  • Complete accurate and timely documentation in the electronic medical record
  • Provide clear nursing handoff reports between shifts
  • Maintain client confidentiality and follow all HIPAA requirements

Qualifications

  • Current and unrestricted Tennessee LPN or RN license required
  • Current CPR/BLS certification or ability to obtain before beginning employment
  • At least two years of nursing experience preferred
  • Previous charge nurse, lead nurse, scheduling, or supervisory experience preferred
  • Behavioral health, psychiatric, or residential-treatment experience strongly preferred
  • Medication-administration and medication-room experience required
  • Strong leadership, organization, communication, and problem-solving skills
  • Ability to create schedules and respond calmly to staffing changes and call-outs
  • Experience with Kipu or another electronic medical record system is a plus
  • Willingness to provide direct nursing care and assist with coverage when needed
  • Must successfully complete all required background checks and pre-employment requirements

Schedule Expectations

This is a full-time leadership position with a primarily daytime schedule. Some flexibility will be required to address nursing call-outs, staffing concerns, training needs, and urgent operational issues. This position may participate in a rotating nursing leadership on-call schedule.

All clinical responsibilities will be assigned according to the selected candidate’s nursing license, demonstrated competency, and applicable scope of practice.
