What We're Looking For
Trillium Health Resources is seeking an experienced Support Transition and Reentry (STAR) Manager to join the Support Transition and Reentry (STAR) Team. STAR is a specialty care team serving incarcerated adults with complex behavioral health conditions. This leadership role provides direct supervision and oversight to ensure the delivery of high-quality, person-centered integrated care. The ideal candidate has experience leading a team, care management expertise, and is familiar with criminal justice systems, reentry programs and community-based services.
On a Typical Day, You Might
Provide direct supervision and support to QP level coordinators, licensed clinicians and certified peer support specialists.
Monitor team's quality of work and take corrective action as needed.
Oversee complex cases ensuring sound decision making and wholistic client care.
Support staff to effectively plan and coordinate member transitions from incarceration to the community.
Collect data, assign cases, and conduct quality audits.
Collaborate with internal departments and external systems including prisons/jails, probation/parole, BH/SU providers, and other stakeholders.
